Canine Physiotherapy Approval
To register your pet with me, the Veterinary Surgeons Act requires approval from your vet, which involves your vet ‘approving’ veterinary physiotherapy. Please do not worry – as this is generally a straightforward process.
Simply fill in the form below, and I will arrange for your vet to provide the necessary approval.
Alternatively, you can download the form to complete at your convenience and take it directly to your vet for completion and return.
Please complete the fields below and click submit
Once you have completed this form, please have a look at availability and book your appointment online.
Unless your pet is currently under the care of another Veterinary Physiotherapist (such as at a Veterinary Hospital), I begin all new clinical cases with a comprehensive head-to-tail assessment.
My consultation process allows me to create a fully personalised treatment plan for your pet, provide tailored exercises and home care advice, and address any questions you may have about your pet's ongoing care.
Every pet is unique, and this initial assessment is essential to ensure that my care achieves the best possible outcomes. You are more than just a number to me – I genuinely care about your pet's results
